The goal of this package is to make it easier for debtors and creditors to understand how reaffirmation forms should be completed and, as a result, to reduce errors. There is a significant overlap of information requested in the Reaffirmation Agreement Cover Sheet and the Reaffirmation documents. To streamline the completion of the forms, a Reaffirmation Agreement Package is available. The Package includes:
- Reaffirmation Documents Questionnaire,
- Reaffirmation Agreement Cover Sheet (B 427), and
- Reaffirmation Documents (B 2400A).
The Reaffirmation Agreement Package are fillable PDF documents that require a PDF application, such as Adobe Reader, in order to open and fill out the documents.
